Program Summary: Full range of outpatient mental health services for children, adolescent and adult residents

Program description

Target population:

Children, adolescents and adults with serious mental illness in need of outpatient mental health care who are on Medical Assistance or who are uninsured or under-insured

Services:

  • Outpatient assessment, diagnostic and treatment services for children, adolescents and adults
  • Psychiatry (children and adults)
  • Medication management and aftercare
  • Psychological testing
  • Individual and group therapy
  • Consultation
  • Co-occurring disorders (MI/CD)
  • Outreach to homeless adults with serious mental illness

 

Conditions of eligibility:

  • Hennepin County resident
  • Diagnosed or suspected mental illness
  • Uninsured or under-insured and/or in target population

 

Referral procedures

How to obtain services:

  • Call 612-596-9438 to schedule an appointment
  • Minnesota Health Care Programs (Medical Assistance, MNCare and all managed care plans) and most private insurance plans are accepted
  • Uninsured clients also can be seen on a sliding-fee basis
  • Clients without insurance are referred to a financial case aide and assisted to apply for health insurance coverage
  • Information needed: Date of birth, address, phone, language/need for interpreter services, insurance name and number
  • Additional information needed for child referral
    • Name and contact information for legal parent/guardian
    • If in foster care, name of foster parent and phone number
  • Additional information helpful if available:
    • Prior history of mental health treatment (diagnosis, previous treatments)
    • Recent hospitalizations (where and when)
    • Recent visits to ER and/or HCMC (where and when)
    • Have you been previously been seen at HC Mental Health Center?
    • Reason for referral: major concerns, purpose of visit, services requested; if court-ordered, next court date

Hours: 8:00 am - 5:00 pm, Monday - Friday. Some later appointments available.
